Full Decode Service Agreement

1. Parties and Acceptance
This Full Decode Service Agreement is between VitalityX Health LLC and you, the buyer identified at checkout. By checking the acceptance box and completing your purchase, you agree to these terms. [PLACEHOLDER — to be supplied by counsel]
2. What's Included
VitalityX Full Decode bundles three diagnostic layers: (a) the Genetic Blueprint (lifelong DNA insights), (b) Epigenetic Age & Health (your biology now, across 11 organ systems), and (c) the VitalityX Blueprint Panel (80+ blood biomarkers). 3. Sample Collection
You will receive at-home collection kits and a lab requisition for your blood draw. Genetic and epigenetic samples are collected by you using cheek-swab and dried-blood-card methods supplied in the kit. The blood draw is performed at any Quest or LabCorp location of your choice. 4. Results Delivery
Final results typically land in your portal within approximately three (3) weeks of receipt of all samples. Turnaround times depend on lab volume and are not guaranteed. You will be notified by email when each report is ready. [PLACEHOLDER — to be supplied by counsel]
5. $500 Membership Credit Mechanic
When your final Full Decode results are delivered, a sixty-day (60-day) clock begins. If you upgrade to any VitalityX membership tier within that window, $500 is applied automatically to your membership. The credit: • Applies only to VitalityX membership signups (not to other purchases) • Cannot be redeemed for cash • Cannot be transferred to another person • Stacks on top of any introductory or promotional pricing If you do not upgrade within the 60-day window, your reports remain yours, but the credit expires.
6. Refund Policy
Full Decode is refundable up to the moment your collection kits are shipped from our warehouse. Once a kit is shipped, the purchase becomes non-refundable due to the nature of biological sample handling and lab partner commitments. [PLACEHOLDER — to be supplied by counsel]
7. Medical Disclaimer

8. Use of Your Sample and Data
Your samples are used solely to generate your Full Decode reports and may be retained per our laboratory partner's standard practices. 9. Laboratory Partners
VitalityX uses third-party CLIA-certified laboratories for genetic, epigenetic, and biomarker analysis. Your samples are processed by these partners under appropriate Business Associate Agreements. [PLACEHOLDER — to be supplied by counsel]
10. Dispute Resolution
